DOT blood pressure rules every CDL driver should understand before their medical exam
If a driver's blood pressure reaches 180 / 110 or higher, they usually cannot receive a DOT medical card that day. The examiner will normally require the driver to lower their blood pressure and return for re-evaluation.
This chart shows how blood pressure levels typically affect CDL medical certification.
| Blood Pressure Reading | Typical DOT Medical Result |
|---|---|
| Below 140 / 90 | Up to a 2‑year DOT medical card |
| 140‑159 / 90‑99 | Usually a 1‑year DOT medical card |
| 160‑179 / 100‑109 | Often a temporary 3‑month DOT card |
| 180 / 110 or higher | Driver usually cannot be certified that day |
Commercial drivers operate large vehicles on public highways. Severe high blood pressure increases the risk of stroke, heart attack, or sudden medical emergencies while driving. Because of this, DOT medical examiners must ensure that drivers are medically safe to operate a commercial vehicle.
